- Dog Diagram or Cat Diagram: To be sure we know exactly where to look for lumps and other concerns, please either (1) print this out, mark the areas of concern, and bring to your appointment, or (2) use an app on your phone to mark the areas, then email or text the picture to us at mail@ovillavet.com or 972-617-9996.
- TCVM/Acupuncture Patient History: If you would like to schedule an exam for your pet to be considered for acupuncture treatments, please fill out this form. Please note that we will call you to schedule the appointment after the doctor has reviewed the form.
